Jörg: Your historical records of the waypoints are probably the key. I don't imagine there's a lot of wiggle room because of the congested airspace, and the need for consistency in ATC.
Have those out-of-state fixes been spelled out in flight plans, or just heard on the radio?
Re Pyramid: It would make sense that it was southwest of the field and perpendicular to the strip so that the aircraft could enter the pattern to either runway.
