Regarding the data center comment, almost two decades ago I attended a infosec training as an intern in one of the national nuclear labs. The facility ran both DoE and DoD projects. One of the DoD guys in the training told us that current guidance from the DoD was to assume that all networks were flat (to assume all were connected to a single point of failure, regardless of actual design), and that they therefore should assume that all their networks were potentially compromised at all times. As much as I hope our infosec position has improved since then, I wouldn't bank on it.
