Looks like a tailored document from a boiler plate template. Lots of stuff there could apply to any air base or USAF test/range facility.

Any idea why it was signed by an Army O-5?

I don't recognize the name? Anyone else ever see it? Also what is "AV" after his rank? Aviation?
